CareConnection: Discharge

  1. Before the day of discharge
    • Verify with clinical consultants that no additional inpatient evaluation or testing is needed
    • Verify with Social Work and Care Management that the patient’s next care setting is ready and willing to accept the patient.
      When appropriate use readmission risk score to determine if patient might benefit from a higher level of care.
    • Complete any transfer paperwork or orders that can be done before discharge (e.g., Home Health orders or documentation, home oxygen)
  2. Day of discharge – in the patient’s room
    • Interview and examine the patient
    • Verify patient has met all discharge criteria
    • Speak with patient and/or family about transport to next care setting
  3. Day of discharge – outside the patient’s room
    • Notify nursing
    • Update patient’s problem list
    • Discharge medication reconciliation
    • When appropriate, ePrescribe medications to patient’s preferred pharmacy
    • Enter follow-up information in patient’s Depart section
    • Notify patient’s primary care provider
    • Enter discharge order
    • Complete discharge summary
    • Update sign-out
    • Complete professional billing
